By accessing the Blended Learning Resources Portal (the “Portal”), you agree to the following terms
and conditions:
- The right to use the programs, services, and resources provided in the Portal is granted only to
the owner of the credential and is not transferable or assignable to any other person or entity.
- The owner of the credential is not allowed to sublicense, lease, sell, or otherwise remarket, or
distribute the Portal’s services, programs, and resources used by Phoenix and Sibs Publishing
House, Inc, by any means and at any time.
- The use of the Portal shall be governed by Philippine laws and the venue for any relevant suit
or action shall be within the jurisdiction of the appropriate courts of Quezon City,
Philippines.
- It is understood that the contents of the Portal are intellectual properties of Phoenix and Sibs
Publishing House. While these contents have been created as a labor of love and are designed to
help teachers and students find success in their respective schools, the Blended Learning
Resources are not in the public domain and are subject to the terms and conditions provided
hereafter.

We at PHOENIX PUBLISHING HOUSE, INC., in line with our "ALAGANG MAHALAGA" culture, extend the same
"alaga" to our education partners with respect to their privacy in compliance with the Data Privacy
Act of 2012 and its Implementing Rules and Regulations. In over 60 years, we have been the partner

We at Phoenix Publishing House Inc and our affiliates (“PPH”) respect the privacy of our customers.
This Notice informs you of the personal data (personal information and sensitive personal
information) that we collect and the way we collect, use, store, disclose (“process”) and protect
your personal data when you visit our website and/or avail of our services (“services”).
PERSONAL INFORMATION

which the identity of an individual is apparent or can be reasonably and directly ascertained by the

certainly identify an individual. While Sensitive Personal Information is any attribute of your
personal information that can discriminate, qualify, or classify you such as age, date of birth,
marital status, government-issued identification numbers, account numbers, and financial
information. Privileged information is any and all forms of information which, under law, constitute
privileged communicated.

Pursuant to the Data Privacy Act of 2012, you have the following rights:
- Right to be informed – you may request the details as to how your personal information is being
processed or have been processed by PPH, including the existence of automated decision-making
and profiling systems;
- Right to access – upon written request, you may deman reasonable access to your personal
information which may include the contents of your processed personal information, the manner of
processing, sources, recipients, and reason of disclosure;
- Right to dispute – you may dispute any inaccuracy or error in your personal information in PPH
systems through our representatives and Data Privacy Officer;
- Right to object – you may suspend, withdraw, and remove your personal information from further
processing upon demand, which includes your right to opt-out of any commercial communication or
advertising purposes of PPH;
- Right to data erasure – based on reasonable grounds, you have the right to order the blocking,
removal, or destruction of your personal data from the PPH system;
- Right to secure data portability – you have the right to obtain your personal information in an
electronic or structured format that is commonly used from PPH;
- Right to file a complaint – you may file your complaint or any concerns with our data privacy

COLLECTION OF PERSONAL INFORMATION
When you avail of our services, we collect personal data which may include: your full name, address,
email address, contact numbers, birthday, and civil status among others.
The collection of your personal data will generally occur in the event that you: visit our stores,
visit or order from our website, join a promotion, participate in a survey, subscribe to our mailing
list, and submit feedback.
We shall only collect your personal information if you voluntarily submit the information to us. If
you choose not to submit your personal information to us, or subsequently withdraw your consent to
allow us to use your personal information, we may not be able to adequately respond to your inquires
or render you our services.

We may use the personal data to provide you our services. Generally, we use this information to:
- process the order/s you placed in our stores, using our website or via phone call.
- respond to specific requests or queries that you may have.

The security measures shall maintain the availability, integrity, and confidientiality of personal

destruction, alteration, and disclosure, as well as any unlawful processing.
We may also share personal data to third parties as may be required by law.
DATA RETENTION AND DISPOSAL
We normally retain personal data related to marketing activities for as long as you accept marketing
communications from us, and we will securely delete such data in accordance with applicable laws
upon request. For personal data that we collect and process for other purposes, we will typically
retain such data for as long as it is necessary to fulfill the purposes outlined in the Privacy
Policy, Cookies Policy, as well as the Terms of Use. Deletion of such data may be done at any time
upon your request.
CHILDREN’S PRIVACY
Our service does not address anyone under the age of 18 (“children”). We do not knowingly collect
personal data from children. If you are a parent or guardian and you are aware that your children
provided us with information, please contact us. Also, if we become aware that we have collected
personal data from children without verification of parental consent, we will take steps to remove
that information from our servers.
